The Capital Medical Center Hospitalist Program is an innovative model of care were primary care doctors and subspecialists have the opportunity to involve Hospitalists, or hospital-based doctors, in the care of their patients.

Q: What is a Hospitalist?
A. Hospitalist are hospital-based doctors, dedicated to the general medical care of hospitalized patients. Hospitalists are familiar with the needs of hospitalized patients and well versed in the management of problems commonly encountered in those patients.
Q: Why is a Hospitalist caring for me?
A: Your own primary care physician may request that a Hospitalist be in charge of your care during your hospital stay. You have the benefit of being seen by a doctor whose practice is entirely focused on the care of patients in the hospital. This may also allow your primary care doctor to be more available to you in the office, rather than trying to balance seeing patients between the office and the hospital.
Q: How do Hospitalists communicate with their patients?
A: Aside from patient care, communication is our most important responsibility. The Hospitalist will be in charge of your care and will see you every day to direct your treatment. The doctor is available to you and your family to answer questions and discuss your care. The Hospitalist will make arrangements for any prescriptions you may need when you are discharged. They will also advise you to make an appoitment with your primary care doctor or other doctors soon after discharge. Our goal is to provide our patients with a smooth and seamless transition from the hospital to their next office visit.
Q: How do Hospitalists communicate with my primary care doctor?
A: The two doctors work together. Your primary care physician can provide information about your past health history to the Hospitalists, and they can discuss significant findings or events. At the time of your admission and discharge, the Hospitalist prepares a detailed report of findings and treatment plans. Those reports are sent to your primary care doctor. While the Hospitalist is in charge of your care during your hospital stay, your primary care doctor is welcome to check on you and discuss your care with you. When you are discharged, you will return to the care of your primary doctor.
Q: What if I need another specialist while in the hospital?
A. Consultations from other physicians are necessary in some cases and the Hospitalists can arrange for these as needed.
