How do I get copies of my medical records?
There are two ways to obtain your medical records. You may call Health Information Management at
(360) 754-5858, ext.1010, or you may visit our office in the hospital. If you visit you will need to bring
your government-issued photo identification and sign a release. You will then receive your records within
48 hours, excluding weekends and holidays.
