Registration Information

Capital Medical Center strives to insure that your visit with us is a pleasant experience. Therefore, we have included this guide to help familiarize you with our registration process before the time of your visit. Upon arrival, you will be asked for demographic and insurance information. You will be requested to sign general consent to treat and insurance authorizations. Medicare recipients will be required to answer the Medicare Secondary Questionnaire and Medicare Rights Statement.

Patient Safety is a top priority at our hospital. Download a personal health information form. Complete the form and present it to your nurse on admission to the hospital. Keep an additional form for your family in case of an emergency.

PLEASE BRING THE FOLLOWING ITEMS:

  • All current health insurance cards
  • Picture ID
  • Prescription/Order for all tests
  • A referral from your physician, if applicable
  • Living Will/Durable Power of Attorney, only applicable if you will be an inpatient. (Please note, you must bring a copy of these forms each time you are admitted.)
  • A note giving "Permission to treat" from parent or legal guardian if the minor is coming without you.

IF YOUR GOING TO BE AN INPATIENT PLEASE DO NOT BRING:

  • Personal valuables including: jewelry, credit cards, checkbooks or cash.
  • Your own medications unless directed by your physician to do so.
  • Cellular phones, as they may not be used inside the facility due to possible interference with monitoring equipment.

REGISTRATION CHECKLIST:

  • Contact your insurance company to receive pre-certification procedure requirements
  • Verify with your physician's office that they have obtained the necessary authorization and referrals for your tests or procedures. The physician's office must initiate the process of authorization.
  • It may be necessary to notify your Primary Care Physician that you will be coming to the Emergency Room for treatment.
  • Registration/Admitting is located through the Outpatient Entrance adjacent to the Emergency Entrance.

Hours of operation: 7:00am- 5:00pm, Monday - Friday.
At any other time, inclusive of holidays, services are provided through the Emergency Entrance 7 days a week, 24 hours a day.
